We here at Sunrise Orthodontics value our patients and put your health and well being first.

We currently offer a Teledentistry app called Dentulu to allow all patients to stay in close communication with our Doctors and Staff. This is a HIPAA compliant platform that allows you to privately communicate and speak directly with us. Please download the application on your android or iphone. You can use the code DENDO8766501 and choose our office from the list of providers.

*Please make sure to turn on Alerts and Notifications in the App Dentulu.

On the Dentulu App you can access live video chats, share photos/videos of problem areas, and chat via text with us free of charge using the above code: DENDO8766501. On a case by case basis, we are able to prescribe necessary medications for pain and/or infections. Sunrise Orthodontics has made this service available free of charge to all patients and their family and friends .

Your first visit – with a preview of what your naturally beautiful smile will look like

Dr. Calkins and her friendly, helpful team always enjoy meeting new patients. Your initial appointment will consist of a thorough examination and a discussion of potential treatment options. This important 30-minute visit will give us insight into your orthodontic needs. We’ll cover the basics of orthodontic treatment and encourage you to ask questions.
Here are some of the questions that we’ll answer:

Do I have a condition that can be treated with orthodontics?
Is treatment needed now or should treatment be delayed, for example, until tooth eruption or further growth.
What treatment procedures will be used to correct my problem?
Will any of my permanent teeth need to be removed?
How long will my treatment take?
How much will it cost?
What are my payment options?

To expedite treatment, we may also take time to prepare your diagnostic records. The records can include:
Cone Beam Computed Tomography (CBCT)
Digital photography
Digital Impression Systems
Doppler Auscultation
These diagnostic records are necessary for developing the appropriate treatment plan. As part of these procedures, Dr. Calkins can provide a preview of the naturally beautiful results you can expect.
We’ll be able to address your treatment needs more specifically after careful analysis of your diagnostic records. We’ll schedule a second visit to discuss treatment options, time frames and financial arrangements. This in-depth diagnosis and planning lead to superior results – and higher patient satisfaction. You’ll leave with a clear understanding of your specific needs, what your treatment will consist of, and how long it will take.
Please assist us by providing the following information at your first visit:

Any panoramic X-ray taken within the past six months.
If you have orthodontic insurance, bring your insurance card. Then we’ll be able to give you an estimate of your costs.

Open Communication

We believe that good orthodontic care begins with open communication. We promise to speak candidly with you about your diagnosis and our recommendations. By working together, we can achieve a true partnership, with one common goal – helping you achieve the beautifully natural smile you were meant to have.

Appointment Cancellation Policy

You may reschedule your appointment with advance notice. But any appointments canceled within 24-hour of the scheduled time will be subject to $60 cancelation fee.

Financial Information

We wish to provide you with affordable, high-quality treatment.
To meet your needs, we offer a variety of payment options. Our office accepts most insurance plans and payment by cash, checks, and Visa, MasterCard and American Express credit cards. We also offer financing options customized to our patients’ needs. If you have any questions about financing or payment, just ask. We’ll explain your choices clearly and work hard to accommodate your needs.

Insurance

Our practice accepts most major credit cards and insurance plans.

Many insurance policies cover orthodontic care. Since Insurance policies vary, we’ll be happy to review your insurance to determine what’s best for you. When you’re treatment starts, we’ll file your claims.
To help us with your insurance filing, please complete the insurance portion of our questionnaire.
If your benefits change during the course of treatment, we’ll let you know. The financially responsible family member is accountable for all charges.
